Standard OpenHCD USB Host Controller (Code 12)

I have a Nvidia 590SLI chipset, but it does seem to be Nvidia drivers. I've
also have a Nvidia graphics geForce 7 graphics card. The problem does appear
to be associated with Nvidia somewhere, most likely the motherboard drivers.

Let's try to narrow it, has everyone installed the Mobile Device Center
update??

Anyone who has recently purchase Vista can submit the problem by email to MS
Tech Support. Be sure to copy the url for these post and submit it to tech
support.

BTW: To TTopJohnson - Click on Accessories, Click on System Tools, Click on
System Information, Click on Edit, Click on Select All, Cliclk on Edit, Click
 
BTW: Nvidia has updates for both their motherboard & graphic boards. Neither
of which solves this problem. One glitch you might find, the graphic board
update on the Nividia website says its 175 but when you downloand and install
it, its 169. Found that the true 175 was on the graphics manufacturer website.

Enjoy using your computer? Then don’t install the optional Vista update “ATI
PCI Express (3GIO) Filter Driver†unless you are certain you own that
particular piece of ATI hardware.
I foolishly installed this bomb of an update, and upon restarting my
computer I found that my network card, sound card, and video card had all
failed.
The fix for this problem is easy. Go to Device Manager, open the “System
devices†node, then right click and Uninstall the “3GIO Filter Driverâ€. Make
sure you click the “Delete the driver from your system†checkbox. Then it’s
just a matter of enjoying the fantastic fireworks of failing drivers. After a
few restarts the correct drivers should be back in use.
I found no useful information online, so here’s the info that Windows Update
provides

I also just discovered this ....uninstalled it and usb is back!... You must
first disable the usb drivers so you can uninstall this......

scary watching the "fireworks" but I'm back in business.
 
By Jove, you guys are brilliant! Thank you so much for finding this out!

I did have some issues w/ the removal of the 3GIO driver coming back. So I
found that when I went into the driver details and hit "Roll Back Driver" it
took care of it too.

If MS is watching this, they should make a very big statement on this update
(triple bold or something) to NOT install it unless you have an ATI system.

Again, thanks so much! Everything works beautifully now!

That also solved the code 12 problem with the Standard Dual Channel PCI IDE
Controller.

I've been attempting to resolve this though HP support. They
recommended I reinstall my system from scratch. Lame answer: HP
Support is getting worse and worse.

Has anyone had any other luck?
 
Please read through the entire thread. Did you attempt this post (see bottom)?

Note: Disable all the offending USB drivers before starting this uninstall.

Well, here is my story in a nutshell.
I followed all the thread and uninstalled ATI PCI Express Filter
(3GIO) I did not understand why that was on my system and neither did
Microsoft!, I uninstalled the USB Microsoft update, and tried the USB
Hotfix suggested on the Microsoft website..
Nothing fixed the problem.
I got on the horn to Microsoft and as a consequence I:
Upgraded in-place - no change (though my system runs faster)
Installed the latest chip set for my 680i motherboard - no change.
Microsoft has escalated this up to the third level.
I will let you know when someone in Microsoft fixes it.
As far as I can see, it is related to the Nvidia motherboards.
Microsoft says they have been getting a lot of calls about this
problem.
All of you should also call and the call is free because it is related
to Windows Update.
The more information they have about this, the better.

I placed a call to Microsoft and after the third escalation, we still
could not find the cause. They were again escalating this to a 4th
tier specialist. In the meantime, the Vista SP! RC1 came out that I
installed. The new Standard OpenHCD USB driver version is
6.0.6001.17052. For those who are having trouble with this, just wait
to get the upgrade or if you are a Technet subscriber, it is available
for download. Clearly, there are problems and issues with the older
version, which caused it to have conflicts for resources.

I have not fully tested the RC1 update fully yet, but so far there are
many things working better and faster, despite what some of the
previous blogs have said about no improved performance.

OpenHCD was part of two USB-Core updates to vista some months ago via
Windows Update, either removing them both or re-installing your OEM
supplied USB drivers should correct the problems.

I freaking love you man, this fixed my problem totally!!!!!
I had the same LAME support from HP recently , and was told to
recover the system to a date before the problem.

THANK GOD I didn't take thier advice on that one!!!

I had almost given up on this recent issue and said the heck with
it,
HATS OFF, YOU RULE!!

TOTALLY FIXED THE PROBLEM!!
