I think much better than maintain hosts file
is run DNSKong ( http://www.pyrenean.com/ ) ,

It is based on white/black lists
of "domain name wildcards"

It can also proxy and cache DNS requests.

Personally, I run

Kerio personal firewall ( sideefect of security is IP add clocking
possibility)
DNSKong ( block domains names )
