More likely the reader driver.
When one does not insert the disk it says no disk.

Sure, but it reserves drive letters for the other slots nonetheless,
even without anything inserted.
It may be just some crude keying (detection) mechanism that makes it
think that there is 'something' there and just because it reserves it a
drive letter it therefor calls it a disk.
When I insert the disk a red light light up,

That may not mean much, it may be just a key on the drive connector.
Does it take a while before it lights or is it immediate?

to format the drive.

You can't usually partition a logical drive and fixed disk
drives are usually partitioned before they can be formatted.
It is even possible that perhaps the disk migh be reconditioned,

Perhaps XP needs some basic information
on the disk which has been damaged....

It should be made to see it without some kind of drive letter emulation
so that it sees it as a physical fixed disk drive.

yeah I think now it's definitely dead. Hmm.... The power inversion seem
to be highly letal
for these USB devices. A "secure" way to destroy any data, just in case
you need to
get rid of some embarassing data :)
