wrong name, date and time in header when printing

When I print from IE the very top line is always the same:

Shelley blahblah, 08:23 PM 1/3/01 -0800, Sponsorship Spreadsheet

It also did this before upgrading to IE 6. The footer at the bottom says
Printed for with the correct name and email, but no time or date.

This header is not printed when printing from other programs. In Internet
Options>Content>My Profile the correct name is listed. Anybody know where
this header information is coming from and how it can be corrected or
eliminated?

IE-File-Page Setup-Header.
The standard header: &w&bPage &p of &P
The standard footer: &u&b&d
