writing amendments to group healthcare coverage

I need to know the correct way to add amendments to a health insurance policy
in writing.
How is it written?

hmmmm....boy are you in the wrong newsgroup...based upon infomation given
(or lack thereof) you need either a Word newsgroup (if you're question is
how to physically add an amendment) or a legal newsgroup.

Cross out what you dont like, and pen in the new and inital the
changes?
