Word Template forms

I created a form in Word for a Doctor's office. How do I
make a template document automatically copy to the next
page when they are done with it. (Dr.'s office wants to
be able fill in the form for a patient, then automatically
have a blank copy of the same form to copy to the next
page so they can start the same form for the next patient.
They have to use the same form about 20 times a day and
want to save it in one document under the date.) How can
you make the form automatically add when you get to the
bottom of one? Thank you.

Templates should be placed in the Template folder. To use the template the
user should go File, New and open a new blank document based on the
template. This should then be saved and another document should be opened
from the template and so on. The original template should never be used for
editing.

A shortcut to the template can be placed on a Toolbar or even on the Desktop
for quick access to a new blank.
