Word is supposed to delete its temporary files when you close the program
normally. They'll be left behind if Word crashes. Sometimes they get left
behind "just because", but I think more often it's something that isn't set
up right -- for example, if Windows has the folder set with permissions for
read and write but not delete.
But if the document itself is being saved in a temp folder and deleted when
you quit Word, then that's probably because you opened an attachment
directly instead of saving to the HD first.
