word 97 form fields

I am constructing a multipage document with a cover sheet
and several continuing pages which have a header and
footer constructed in table format. each cell is dedicated
to one of the form fields shown on the cover sheet.

On the cover sheet I want to enter information in form
fields (eg client name, quote number etc.
What I wish to do is for this entered text to be
replicated within the header & footer in all subsequent
sheets.

The completed document will be protected and will only be
availble for text entry in either the form fields or in
areas where a continous section break has been entered.

Can anyone please help me here??

You could insert cross-references to the Form Fields using
Insert, Crossreference and the reference type being
Bookmark.

The one thing that I have found with this type of setup is
that the header&footer do not update until repagination or
on print (print preview as well) so I am not sure if this
is the best solution.
 
To add to what Tara has said, note that every form field has a built-in
bookmark, which is accessible in the Form Field Options dialog in case you
want to modify it. If you check the "Calculate on exit" box in each form
field, the cross-references will be updated automatically (though I'm not
sure if that applies to the ones in the header/footer).
