why does a new add-on go directly to disabled items in powerpoint?

I installed a new add-on to PowerPoint but when I launched PowerPoint, it
wasn't there. After searching for a while, I found it in disabled items. What
are the rules for automatically disabling an add-on in PowerPoint? I didn't
even get a warning message from PowerPoint that it was disabled.
 
What does you add-in try to do? Did it crash for any reason after you loaded
the add-in? In almost all cases PowerPoint does not disable it
automatically. It will prompt the user and ask for confirmation before
disabling the add-in.
