Why do custom contact templates revert back to the default ones p.

At our office we run Office 2003 and MS Exchange 2003 w/ SP1.
A couple of our users have created custom templates for their contacts but
inevitably the contacts revert back to the default template.
Most often too, the custom template disappears preventing further access to
it.

What can be causing this??
 
Have you applied Office 2003 SP1 and cleared the forms cache after
installing the SP?
 
Office is up-to-date including SP1 but I havn't cleared out the forms cache.
Where is this located?
Right now she has her custom template working and is checking it everyday to
see if it reverts back to the default one again.
 
Choose Tools | Options | Other | Advanced Options | Custom Forms, then click
Manage Forms, then Clear Cache.

It erases the cache copies of all your custom forms. The cache will reload
the next time your invoke the published form either by creating a new item
or opening an existing item.

Is what I described happening a known issue?
This user is anxious to use Outlook with this custom template but is real
wary of entering her many contacts and having it revert to the default again.
Are there any settings or steps that can be done to ensure this won't happen
again?

Yes. Clearing the cache is an essential step after applying SP1, which fixes
most of the forms cache issues in Outlook 2003.
