Well, I've never used it myself yet, but take a look at Oubliette:

If it's of similar quality as Keynote (from the same developer),
then it should be an *excellent* program.

.) if it is part of an 'login/passwd' or registration

.) if it's somehow beside that related with the account (i.e. for a
contact if the account is for software serials, or a website)
If it's important and/or makes sense for the account, I use it.
It's primary up to the user if fields are filled or not.
PINs, Secure Password Manager.

self-contained, so I can run it anywhere from a USB drive.
