Volume auto decreases to nothing

The volume automatically lowers itself in the
equalizer/volume adjuster to the lowest point. It happens
every time a program or new window is opened. If I try to
manually raise the volume bar it fights against it and
goes back to the bottom as soon as I unclick the mouse. I
am able to get it to stop automatically lowering by
playing around in the properties of the equalizer, so that
I can raise the sound bar without it readjusting on it's
own. The problem is, the minute I open another window,
the problem happens all over again.

I have no idea what settings/drivers/applications I need
to adjust. The computer has hardly anything on it, just
about 4 programs on it is all. The volume used to work,
now it doesn't.

Update the sound card drivers on the machine. What coincided with this
happening? Any new applications installed?
