voice to voice over IP programs

I am using Picophone for chatting to my buddy. But I am getting fed up with
at times very slow transmissions - also I can hear buddy but buddy can't
hear me or gets a very broken up transmission. I know it may not be
Picophone's fault that things are not perfect but none the less are there
any programs like picophone out there only better. Ideally I am looking for
a clear transmission, no delay, no stutter and a program that is not
affected by another program suddenly running like for example an email
checker doing its checking, or say a firewall doing its thing. Someone will
probably suggest Yahoo or the like but I only want contact with my one buddy
and not be subject to loads of popup messages from people I do not know or
even wish to know.

Good question. I am after a similar solution but with video as well. Any takers?

The 2 main standardized protocols used for VoIP (Voice over IP)
applications are "SIP" and "H.323". You can use that in your searches...
