VERY URGENT - Could not install the custom actions

I'm tying to use in OL2003 a form created with OL2000. When I try to open the
form, I get the errore message "Could Not Install The Custom Actions"; then I
click OK and the form seems to work correctly.
I've already try to clear the cache and republish the form, but the error is
the same. I've also tried to call Microsoft Support and they sent me an
OL2003 post-sp1 hotfix (see KB 839629), but nothing changed.
Any suggestions?

In design mode, go to the (actions) page and remove the custom actions
from the form.
