Using Forms

I'm trying to create a form that has check boxes and areas to put comments.
I can add a check box from the forms menu but when I add a text box, it adds
a small box. How can I underline that box so it looks like a line for users
to enter data. Also, how can I expand that to look like a comment section?
Is this possible?

The usual way is to set up your form in a table with a border at the bottom
of selected cells where the user is to type the input.
