Using Date Created field

I have searched Google and this ng for the answer but haven't found it.
I have created a form for use in the Contacts folder. Works well. My
only problem is, I would like to add a field to the form showing the
date the Contact was created - added to the Contacts folder.
I can find a 'Created' field but I can not seem to get it to 'stick' to
the form. I have tried to add the field 8-10 times (literally) but I
can't get it to stick. All other changes seem to appear the way I add them.
Is it possible to add a 'created' date field to the form?
If so, which field do I use?
How do I get the change to 'stick' to the published form?
 
All you have to do is drag the Created field from the Field Chooser to any
custom page, then publish the form. Remember: The date is set only after the
item is saved. You will see None as the date on a newly created item. If you
save and close the item, then reopen it, you should see the Created date on
the form just fine, in the control created when you dragged the Created field
to the form page.
 
After several attempts, I finally got the date to appear . . . almost.
In using the 'Creation Time' field from 'All Document Fields.' I now
have the field showing in the published form but the 'None' appears
eternal - saving, editing, nothing seems to get it to show a date.
 
You should be using the Created field from the list of contact fields.

Very good. Thanks for the help.
One bonus question. Back when I added forms to Outlook 2000, I
'attached' forms to specific folders. THat is, in Contacts, I could hit
CTRL-N and the my custom form would popup - not the Outlook default. I
can't remember how I did this and have, so far, failed to find the
answer on the net. Please direct me to the proper site.
 
You would need to modify the folder, using its Properties dialog, to make
your custom form the default form for that folder.

Shows how smart I am. I kept looking in the options in Design Form.
