Why do I have users with
tsmith
tsmith.001
tsmith.002
etc.?

What causes it to do this on the local workstation?

Someone correct me if i'm wrong but I believe this is due to a profile
being created with the same name and same domain but with different SIDs
so Windows will normally create a profile directory in the form of
username.domain or if not on a domain then it is username.hostname but
if it already exists for that user then a number is appended to it so as
to not overwrite the original. This has happened to me when I had to
reinstall XP and I used the same username/hostname during setup and it
ended up creating another directory like what you have seen.
