Hi Lee,
The great majority of the times we've seen this behavior it usually gets
resolved by one of the following:
1. After a reboot, to allow the PendingFileRename operation to complete.
2. If a third party application is not allowing the PendingFileRename to
complete, like a good example anti-virus has a handle locked on a file.
Then you can use the MSCONFIG utility from Windows XP to disable all the 3rd
party services and programs on the server. Reboot and reinstalled the SP4
and/or patches.
3. Find a known working server that the same patches were applied to, then
in Safe Mode rename and replace the Userenv.dll and reboot to resolve the
issue.
