I have my web site on a server with front page extensions.

Ideally, I'd like clients to fill out a form on the internet prior to
coming to the office, and submit the entire form electronically. I don't
want the text bits and pieces e-mailed to me like an ordinary form, but
ideally want the questions preserved as well. Is there a way to use a pdf
file on the web site, but have the user able to modify the template form and
then submit the pdf form via e-mail or file? I have adobe professional. Any
siggestions on how to accomlish this otherwise?
 
Formatted html emails are not possible w/o server side scripting
Yes you can create a downloadable PDF form (w/ fillable fields) they could download, fill in and email
see the adobe site for info

You could do up the online form and have the data submitted to a database
(using the database results wizard). (uses Ms Access) then once the survey
period is up, download the mdb file, open in Access and create a report of
the data according to the normal useage of Access.

You could also display the results using the DRW on your web site.

Otherwise, yes, you could also create a survey in Word (or acrobat) and make
a PDF for downloading and mailing back to you.

If you use the PDF 'online forms' type feature, I believe users can
download, fill in the form and then fax/email/print/mail as normal, if you
had the appropriate fax software for example, but otherwise by email or
post.
