USB ports

I have a desktop pc that is equipped with a couple of USB ports. In
addition, I have an add-on USB 2.0 card installed. The USB ports have a
strange behaviour that once I upplug any USB device on USB port of pc or
add-on card, it will not work again if I re-plug it onto pc unless I reboot
the pc. Can someone advise me any possible causes and fixes to this issue.
