How can I disable all usb ports on a workstation to prevent users from
plugging usb devices such as flash drives and others into the ports? I want
to secure the worksattions from disgrutle employees possibly copying data
from a computer and taking it with them when they leave the company.

Can you disable them in the bios and then password-protect the bios?
 
How can I determine what the ports are? I have several ports on my pc's. I
have Intel(R) 82801DB/DBM USB Universal Host Controller - 24C2 and Intel(R)
and 82801DB/DBM USB Universal Host Controller - 24C4 which I assume are two
of the ports. I have four ports on my PC two in the front and two on the
back. How can I distinguish them in Device Manger? I also see two USB Root
Hub, USB 2.0 Root Hub, and Intel PCI to USB Enhanced Host Controller? Should
I just disable everything?
 
If you want to specifically identify each and every one, disable one at
time and see what connection stops working.
 
Why go halfway? Desolder them from the MB. :)

Depending on the motherboard, you may be able to disable them with a
jumper, or in the BIOS. If you use the jumper method, you can probably
clip one of the pins on it, so the deed can't easily be undone. If you
do it in the BIOS, even passworded, it's pretty simple to clear the
password.

Another physical solution involves sacrificing a few usb cables, remove
the connector, push it in, it won't come out. :)
 
you still need a third-party product like devicelock to solve this
problem even on W2K3 R2. Moreover, programs like devicelock provide a
lot of additional functions such as: auditing, white listing, etc.
