USB Blue Screen of Death Problem

I'm running Windows XP, latest service packs, fixes, etc. Whenever I insert
a USB device in the machine and then take it back out (using the menu to stop
it or not), I get the blue screen of death. Have searched the Knowledge Base
and google to no avail. Can anyone tell me the steps to figure out what's
wrong?
