This is the procedure that Microsoft replied to my inquiry about not being
able to open forms in Outlook 2003.
Step 1:
========
Start Outlook in Safe Mode
---------------------------------
Safe Mode will disable all add-ins and extensions that will not start, as
well as any corrupted resources, files, registries and templates.
Note: Disable all of the anti-virus programs and the email monitors when you
perform the following steps.
(a) Close Outlook.
(b) Go the Start menu of the Windows. Click Start menu-> Run, type "Outlook
/Safe"
(without the quotation marks and there is a space between Outlook and /Safe)
in the Open box and press Enter.
Step 2:
========
Disable Add-Ins in Outlook Safe Mode
---------------------------------
1. Start Outlook in Safe Mode, click Tools -> Options, and then select the
Other tab.
2. Click the Advanced Options button, and then click Add-In Manager.
3. Check the first item, and then click OK (this step is very important).
4. Click Add-In Manager again, and then ensure Uncheck all the items, click
OK.
5. Click COM Add-Ins, and then remove all the items if it appears.
6. Click OK until the Options dialog box is closed, then exit Outlook, and
restart your computer.
7. Run Outlook in Normal Mode.
This should fix the problem.
