Outlook Unable to Add Business Contact History: Microsoft Office Outlook 2003

For my contacts, I use Microsoft Office Outlook 2003, Microsoft Office Professional Edition, 2003.

In my "Business Contacts", I am no longer able to "Add" a "Business Contact History".

What are your recommendations?

I have tried Choose Tools | Options | Other | Advanced Options | Custom Forms | Manage Forms | Clear Cache

I am still unable to to "Add" a "Business Contact History".
