Trying to work from home via UltraVNC

First of all, thanks to so many of you who have helped in the past.

I am trying to work from home via UltraVNC and want to be able to
access a computer at work that accesses the internet through our
server.

I did whatismyip.com and got one IP address, and did ipconfig from the
cmd line and got another. Which one do I type in on the client to
access the destination computer?

Do I need to open up a port?

How does your office PC connect to the Internet? Via an
ADSL router? Dial-up connection? Do you have a firewall
at work? Do you have a fixed external IP address at work?
