I see utilities everywhere claim that Windows can not uninstall programs
properly. They claim pieces are always left in the registry by the ARP.
How true is this? Is the only work around to use system restore
before every app install? How big a performance or stability hit does
this really cause?

Gary,

It depends upon the uninstaller suite [of that particular program]. Some uninstallers [built-in with the application] remove the traces completely, and some don't [as designed by the manufacturer]. And yes, System restore is recommended before installing an application. A System Restore snapshot will not affect your system performance, but it takes considerable amout of disk space which you don't need to worry if you have a large capacity HDD installed.

restore point when an installation starts?
