I'm pretty sure either RTCW or MOHAA activly supported this, are there
any other games which use this apparently underused feature?

I think at least one of the Serious Sam series games supports it, albeit
partially, and there are patches available for a few other games.

UT 2003 supports it. You enable it in the .ini file.

So how does one turn on Truform in UT2003? I saw no mention of it in
the ini file.

In ut2003.ini, at least on my system, in the Direct3d section are
these 2 lines:

UseNPatches=False
TesselationFactor=1.000000

To turn in on, set the first line to True. Second line adjusts it.
It does improve the look of the models in close up situations. Really
only recommended for the 8500's (and I think 9000's, 9100's and
9200's). The newer models (9500 and up) do TruForm in software which
takes a considerable perfomance hit.
