AntiSpyware identifies a trojan
(Trojan.BHO.NameShifter.DH) which it successfully
removes, but which reinstalls itself upon startup/reboot.

I've tried to remove this in safe mode as well. no luck.

Any tips? Can anyone direct me to a relevant post? It's
very frustrating?

==> What are you trying to remove it with? AS proggies in general are
not written to deal with trojans. You need to use an anti trijan
scanner. There are several good ones available that offer a fully
functional 30 day trial.

See: Tauscan at; <www.agnitum.com> ; Ewido at;
<http://www.ewido.net/en/> ; a² Free at;
<http://www.emsisoft.com/en/software/free/>

These are 3 of the best 5. The other 2 being TrojanHunter at;
<http://www.misec.net/trojanhunter/> and TDS-3 at;

Try Ewido, make sure you update the def files then scan from Safe
mode.
