Track date you entering appointments into calendar

After furious head scratching, WendyB asked:

My boss has a tendancy to enter things and then she states "I have no idea
when I put in on my calender."

I am at my wits end.

Display the calendar in a table view and add the "Created" date to the
header line using Field Chooser.
