to uninstall unremovable update

"Once you have installed this item, it cannot be removed"

This question is only for those who know where is this phrase from

Well, it is from installing of some hotfixes, updates, service packs to
Windows 2000/ XP and its applications.

What does this phrase exactly mean?
Why update cannot be uninstalled?
What to do if it causes problems (clean install of Windows)?
Can it be uninstalled by returning to previous Restore Point thru System
Restore (Windows XP) or thru Goback (in Windows 2000)?

Does this phrase mean that I cannot use migration wizard, and this
unremovable item would migrate after to "clean" or re-installation of
Windows?
 
It means exactly what it says.
There is no option is Add/Remove Programs for that patch as there is
for most other patches.
System Restore is the normal way to remove one of those patches.
Go-back will also work with both Windows XP and Windows 2000 as well
as other operating systems.

Patches are a part of the operating system and will need to be
reinstalled after a Windows reinstall whether a Clean or Repair
Installation.
 
More precisely, I would like to know which, how, where changes are made.
I am not sure about repair reinstallation. I have multiple experience that
the problems are preserved with repair installation or installation over
existing Windows
 
The precise locations and other details of changes is dependant on the
specific patch.
A patch for Windows Media Player will probably have changes within the
media player files and maybe no where else, similarly other patches
will affect changes to their respective data.
Look at the documentation of the specific patch for specific file
details.

If you want to guarantee something is gone, a Clean Installation after
deleting the partitions is the way to go:
