To send internal & external mail simultaneously

I have a problem while sending internal & external mail together. I have
exchange server 2003 but no fixed I.P. address so we rely on pop3 server.

* When i send using pop3 server as default internal mails
wont go
* When i send exchange server as default the recipent
recives internal e-mail address and he cannot reply.

Fixed IP has no relation to the ability to send using Exchange.
