Third mouse button emulator?

I have the Kensington Slimblade mouse/keyboard for portability, but
there is no third mouse button. Is there a third mouse button emulator
around?

If you have a two buttons mouse and you would like to use a third than this
utility is very helpful on Windows systems. Pressing Ctrl or
Shift(Selectable) together with right mouse botton will emulate a press on
the third(middle) mouse button.
System requirements:
- .NET Framework 2.0

I actually saw that before I posted - but the author pulled it from
his site, so the links I see are dead...
I don't know of a 'trustworthy' download site which has it. I emailed
the author.

Have you tried clicking both buttons simultaneously?
 
Yep.. it doesn't do anything... and Kensington agrees that their
driver doesn't have a setting to emulate it...
No response yet from the author of the previously mentioned program.
 
The author said that he no longer had any use for the program and
doesn't have a copy of it anymore.
'It probably can be found via google' --- except for the fact that I
have yet to find a download source which I trust.
