the name in my calendar under "last modified" is wrong

If I open an appointment in my outlook calendar the name at the bottom left
that says "last modified by...." is not my correct name
 
I am also experiencing this problem with one of my users. I have verified
that all of the settings in the profile are correct. Any other information?

Version of Exchange? Did either (or both) acct move to a new Exchange
server?

Exchange 2007, Outlook 2007. I don't think the account was moved to a new
Exchange server, but can not verify that information. The account may have
been moved during a migration from Exchange 2003 to Exchange 2007 but I was
not here when this took place and don't know if the user was or not.
