The caps lock is stuck "on" on your keyboard.
The copy icon on the toolbar shows two documents.
Hint: When you select desired document(s) you
press 'copy' button and change some other folder and
press 'paste' button. Bang...copied.
If you do not want/need the copy icon use view-toolbars-
customize to remove it.
