testbox hyperlink

so I want someone to beable to type in the subpage extention into a text box
and it will go to the subpage.
Here is the page I have created to do this

Problem is I have not correctly done this (hernandez) is the first subpage I
have tried to do this with.
Does anyone know what I have done wrong?
 
FP form do not support your
<formaction="default.htm" method=get></formaction="default.htm">

You will need to create your form and form processing with server side scripting (and probably a database) supported by your host OS
