Telephone Message Template - Outlook 2003

Does anyone have a template for taking telephone messages that works in Outlook 2003. I don't want to create one if someone has one that they'd be happy to share with me.

Tested with Outlook 2002 and 2003. Works like a champ here for quite some

Outlook 97 had a "Value Pack" with various forms; see

still work.

"While You Were Out" can be downloaded at

The one suggested by Siegfried Weber seems to based on the above; it
also seems a bit more comprehensive.

Siegfried, thank you so much for giving me the link to your template - I have installed it and it is working wonderfully - you have saved me a considerable amount of time and obviously unnecessary effort. May I share this template with other people
