Telephone Icon to show in recipient's mail box

I have managed to create a Telephone Message form in Outlook 2003 and
to place an icon for it on the toolbar but I would like to create a
telephone icon to show in the recipient's mail box when I send the
Telephone Message through.

The icon for a custom form is set on the (Properties) page of the form in design mode.
