Tables and form tool bar

Hi,

I am creating a document that consists of multiple tables
that contain text form fields. These tables span over
several pages. The document contains a header and a
footer already.

We are trying to accomplish the following: We would like
there to be a 'header-like' line near the top of ever page
that contains text form fields. This same information
needs to be displayed near the top of each subsequent page
(So we would probably need a link of some sort).
Any suggestions on how we may be able to achieve this?

If the fields will have the same content on each page, put the actual form
fields on the first page for users to fill in. On the Layout tab of Page
Setup, check "Different first page" so that you have a separate Header and
First Page Header. Insert cross-references to the form fields in the Header.
