Table of Contents and Idexing

Can someone please help? Having created over 50 forms for
multiple users to access, it is very cumbersome without a
table of contents and index, but I am lost as to how to
make that happen. I have tried to make it work with a
PivotTable system, but I can not make that work correctly
either. Does any one have some ideas that might help?
