Table in Relationships Twice

I have a table "Program Inventory". When I open Relationships, I see the
table twice - as "Program Inventory" and as "Program Inventory_1"

Both are linked to "Policy Inventory" at the same point.

If I remove the "Program Inventory_1" from the relationship window, close
everything down, cold boot the machine, it comes back.

Why is there a second "Program Inventory" in the relationship and how I can
get rid of it? Did I fat finger something?
 
Perhaps you used the Lookup Wizard in your table. Access will create a
second relationship, even if one already exists.

You can get rid of the second one by deleting the relationship first. i.e.
Click on the line between Program Inventory_1 and Policy Inventory. Then
hit the delete key to delete the relationship. Then you can remove the
Program Inventory_1 table.
