Tab appears when updating Table of Contents, esp. with TOC 2 style

When updating a TOC, especially if it's TOC 2 style, a tab
appears each time. There is no logical reason for a 2.25"
tab appearing each and every time the Table of Contents is
updated. The tab itself does not appear within the style
before or after the update even though it appears on the
screen.
Is this a bug in Word? Any suggestions on how to fix it?

I seem to recall seeing something about this, once or twice.

Which version of Word are we dealing with? Is numbering used
with the style TOC 2 is picking up, or does this style
contain the tab?
