Just delete the Switchboard Form (or rename it as a Back-up) then re-create
the Switchboard using the Switchboard Manager in Tools / Database Utilities.
IIRC, the Switchboard Manager will create the (new) Switchboard Form but
will use existing items in the [Switchboard Items] Table.
