Switchboard disappeared

I created a Switchboard in Switchboard Manager but it isn't in Forms. I DID
delete something called Switchboard in Forms because I did the first version
all wrong. What do I have to do to get the Switchboard back in Forms?
 
Sorry. I should make clear that I after deleteing Switchboard from Forms I
continued to work on it in the Switchboard Manager, but my continued efforts
are not showing up in Forms.
