Stuck on how to design this

Anybody know of any pictorial resources for Forms
development? I am trying to configure a specific section
on a Form and I am not sure how to make it great.

The goal is to offer a list of names to choose from. Once
a name is selected, there are several other selections to
be finalized related to the entry of the person. Those
other selections are defaulted based on who it is and
other factors, but may be modified. Once that is
finalized, the person and their related information should
be added to a table.

I would also like the ability to edit a previously-entered
name.

So I'm stuck on how to go about this configuration. I have
some ideas but they are not that great.

Does anybody know about an online resource with a ton of
screenshots of userforms? Looking at something may trigger
some ideas. Of course if anybody has any ideas about this
I'm all ears.
