Strange & unusual problem with icons in XP

I've got a problem here that I haven't seen before, so maybe someone
out there can help...

I'm running XP Pro, and lately the icons for all executables and
application have "disappeared"! I can still change the shortcuts on
some icons, but it seems that the "default" icon for any application
(the one that appears as the first one when you try to change the icon
for that application) will only show up as a missing icon in all
explorer windows.
I'm guessing Icon Packager or something else has screwed up the reg,
but an uninstall of said application + others didn't help...
Any ideas where this sort of thing is stored in the registry, or any
other solutions?

(I've already tried refresing the cache, deleting the shellicon.db and
so on... the usual stuff...)
