Have you tried scanning in Safemode?
How about using the Advance Tools, System Explorer,
Startup Programs, and remove unknown (if you have done
the research on them), removed known harmful startup
items, and even removed unnecessary startup times,
restarted the computer, then scanned for spyware? It
could help as well has help with the system's
performance.
www.castlecops.com/StartupList.html is a good start for
startup item information.
- just put the last part of the startup item path in the
search bar such as "explorer.exe" and it "may" give you
some information. If it doesn't then do a second search
at Google.com. If that doesn't work then it most likely
is just a randomly named item that could be removed.

I am quite happy that this tool does have BHO and Startup
Item removal all in one. I use to use Hijackthis for
startup item, toolbar, and BHO removal or Startup Control
Panel to remove startup items. I must say that I am
quite impressed. I would personally be much more happier
to hear if Microsoft makes this program free as a Windows
Update so that it would be much more easier to work on
spyware cases on the floor. (yes I am a Microsoft
support rep - and damn proud of it!)
