Is there an utility which shows not only the startup programs, but also the
exact order they are started?

Hmm..I'm not sure of how Windows decides what gets loaded first, but I
do know of a way of controlling the order using a program called start
up Delayer [http://www.r2.com.au/index.php] It lists all the programs
that start when Windows does and allows you to assign an order, how long
to wait until Windows has loaded before launching a program, and some
other cool options. It fixed a conflict I was having between two programs.

StartMan is exactly what you want - I just discovered it recently, but
it's become my favourite startup manager already. It's default view is
the starting order of all progs, and then you can go to the various
groups.

includes a screenshot; unfortunately the home page no longer exists) -

StartMan is an extended 'msconfig' Startup Manager, designed to take
control of all those pesky programs and background services that load and
run at logon - most of which you probably don't need. As well as
providing the usual trouble-shooting options to enable or disable
startups, StartMan also permits the removal of startups.
Duplicate Detection and Removal, a unique feature of StartMan, can be
configured to remove all duplicates automatically, with or without
prompting.
Orphan Detection and Removal. As above, for orphaned startups.
Executable Detection and Removal. As above, for executables.
Includes a full range of sorting options to make it easier to locate a
specific startup - by name, by filename - even by command line parameter!

"Autoruns" gives you a list, sorted in time order, of all programs and
processes which are set to start at reboot.

You can delete any program from the autostart list.

While you are at this web site, their Process Explorer is the best
process explorer too.

I use it 4-5 times a day, to check what processes are running, and
stopping the processes I do not want running.

When I see a new program show up in process explorer I find the exe file
it comes from. I check if it autostarts in autoruns.exe.

This way I can see what processes are running, and kill the processes I
want to stop.
It makes it possible to keep an eye on all kinds of viruses, scripts,
trojans, adware, spyware, etc..
It also gives a lot of valuable information about how different programs
handles common system resources, for example.
