Open a folder, click Tools > Folder Options > File Types > Folder > Advanced
You can add your new context menu item and it's command there.

May I am blind, but I ran regedit and looked in both
HKEY_CLASSES_ROOT\Directory\shell and HKEY_CLASSES_ROOT\Folders\shell and I
did not see anything related to the start menu options.
Also I did not have a clue about how to use Folder Options, although I did
look at it. If you could elaborate on these procedures, I would sure
appreciate it.

You may add your entries here:
HKEY_CLASSES_ROOT\Directory\shell
or
HKEY_CLASSES_ROOT\Folders\shell
Affects all the folders. You may use the Folder Options > File Types dialog
as well. Once all is done, type "regsvr32 /i shell32" in the RUN box.

When the Start Button on Windows XP Pro is Right-Clicked, a list of options
such as Explorer, Search etc appear. I would like to know how to add to and
delete from that list of options.
Any help is appreciated!!
