Standard intake form for new database

Does anybody know where I can download an intake form with the most necessary
questions I have to ask when a client wants to have a new database. I can
think of the following questions to ask:
By how many users will the database be used.
How many data is to be expected
Of course questions about the look of the forms and reports
And so there are more questions I can think of but perhaps there is already
a standard intake form.

What you are looking for is called specifications. Each devlopper has their
own way of doing this. If you Google somthing like database specifications,
you'll find tons of stuff on the subject. You'll see very rapidly that it
can be very complex depending on the level of detail you wish to track...
this said more detail does prevent issues from arising later down the line,
so it's a fine line that you'll need to define for yourself.

Thanks for your answer. But what I am interested in is what are the common
questions you have to make when you have your first interview with the client
to design a complete new database. What kind of requirements can I expect
from the client.
