Simple Order Form

Greetings,

I have searched and googled - still nothing so I turn to you.

I need to create a simple order form that will add up the purchases,
that's it! That form would be emailed to me and I would manually
input the order in another system. Not the best way to go, but
cheapest and easiest right now - I take maximum 20 orders a month.

How can I do that in FrontPage?

If I can't, can I buy something cheap? I really don't need a Miva or
such.
